Resident Feedback Form

This form can be used to report issues of concern to the GME Office. It may be filled out anonymously. If you choose to provide your name and contact information that will enable us to respond to you. We will keep your identity confidential to the extent allowable by Upstate policies and local, state and federal laws. Confidential issues related to bias, discrimination or harassment will be shared with the Office of Institutional Equity (OIE). OIE is responsible for reviewing and, when appropriate, objectively investigating complaints of discrimination and harassment based on a protected class.
